I certify that each answer to questions and all other information set forth in this application are true and complete. I further represent that such answers and information and other information submitted with this application are a full and complete disclosure of my knowledge regarding the information sought by New Haven Community Schools. (“NHCS”) I understand that any incorrect, incomplete or false statements or information furnished by me may void this application or subject me to discharge at any time after employment with NHCS. 
 
I authorize the NHCS to make any investigations of my personal or employment history and authorize all former employers, including NHCS any person, firm, corporation, credit agency or government agency to give any information they may have regarding me and my work performance to NHCS. 
 
I authorize my current and former employers to disclose to NHCS all requested information whether or not substantiated in my personnel record, including, but not limited to, any information concerning unprofessional conduct by me, and to make available to NHCS copies of all documents, whether or not maintained in my personnel record, including, but not limited to documents relating to my unprofessional conduct. 

I release NHCS and its employees, agents and all of my former and current employers, educational institutions and other references I have provided, from all liability and damages for releasing, in good faith, or using information concerning me and my performance record. I waive any right under the Bullard-Plawecki Right to Know Act, 1978 PA 397, to receive written notice from NHCS or any former or current employer that disciplinary reports, letters of reprimand or other disciplinary action taken against me while employed will be or have been disclosed to a third person or entity. 

I understand NHCS may conduct or have conducted by an individual of their choice a criminal history background search on, as required by law or as permitted by law. I consent to this search being conducted and release NHCS and its employees from all liability, claims and damages related to conducting the search and/or use of the information from the search. I understand that criminal convictions may result in disqualification from employment with NHCS or dismissal in employment if an offer has been made and accepted. 

I understand and agree that, subject to any collective bargaining agreement or individual employment contract applicable to me, my employment is at-will and can be terminated with or without cause at either my option or NHCS. I understand and agree that no supervisor, agent, or employee of NHCS, other than the Board of Education or that individual vested with the authority by the Board of Education as stated in Board policy, has now or has had in the past any authority to enter into any agreement for employment for any specified period of time or to make any agreement that is contrary to or in modification of the above described employment relationship and that any such agreement or representation must be in writing and signed by the Board of Education and me in order to be effective. 

I understand that, if employed, additional personal data will be required for determination of eligibility and for statistical purposes.
